How IntellxxDNA handles Patient Data
What information does IntellxxDNA have about patients and who is it shared with?
IntellxxDNA provides services that help your clinician diagnose and treat you as a patient. In order to use IntellxxDNA’s services, your clinician arranges for you to provide a saliva sample to be sent to a laboratory for testing. The laboratory sends the test results to IntellxxDNA and we use the test results to create reports that help your clinician understand how your test results can help them to diagnose and treat you.
We may also have information that you or your clinician provides to us, such as information that identifies you (like your name and contact details) and payment information.
IntellxxDNA shares information about you with your clinician. For example, when your clinician(s) order one or more reports from us, we include your relevant test results that we received from the laboratory in those reports that are shared with your clinician and your clinician’s office. We may also share your information with a limited number of third parties that have a need to access your information in order for us to perform our services for your clinician, such as our technology providers. In limited situations, we may also be legally required to make your information available to regulatory authorities, law enforcement, courts and similar mandatory disclosures.
We protect the information that we have about you in accordance with applicable laws and regulations. We also have entered into a strict confidentiality agreement with your clinician to protect your personal health information and we require any third parties that may have access to your information to enter into a similar strict confidentiality agreement with IntellxxDNA.

What happens to my data after IntellxxDNA provides the report to my clinician?
Your data is stored securely so that the IntellxxDNA services can continue to support your clinician’s current care for you and also to allow for your clinician to request report updates and additional reports as scientific understanding evolves.
You have the right under the Patient Agreement that you sign with IntellxxDNA to de-activate your IntellxxDNA account. If you choose to de-activate your account with IntellxxDNA, your data will no longer be available to you or your clinician at all. This means that, if your clinician or future clinician wants to access IntellxxDNA services for your treatment, you will need to purchase IntellxxDNA services again and will need to submit a saliva sample for new testing. IntellxxDNA may retain information regarding deactivated accounts as necessary to meet its legal and compliance requirements.

What happens if the company changes ownership?
Like other healthcare and technology companies, if IntellxxDNA is acquired or undergoes a major business change, data may be transferred to a new owner.
In that case, your data would continue to be protected under applicable privacy and security laws, your Patient Agreement, and your existing choices (such as whether you authorize research participation) would still apply.
Will IntellxxDNA use my data for research or education?
No, unless you give a specific written consent to have your data included in research or education.
When your clinician requests IntellxxDNA services for you as their patient, they sign a clinician services agreement with us and you also sign a Patient Agreement with us. These contracts limit how IntellxxDNA can use your data. They do not permit us to use your information for research purposes.
However, we know that there may be situations where you or your clinician may want your information to be used in research or education. If this happens, IntellxxDNA can only use your data for research/education if you have signed a special written authorization form. This form requires IntellxxDNA to “de-identify” your data before it is used for research/education. This means that your name and other personal details are removed from the data that is shared with researchers/educators so that the data provided for research or education is no longer directly tied to you as an individual. De-identifying your information in this way allows researchers to study patterns without knowing who the data belongs to. The special authorization form also allows you to specify the specific purposes for which your data can be used. We can provide you with this special authorization form.
If you have signed the research/education authorization form and later decide that you no longer want your de-identified information to be included in future research/education, you can withdraw your authorization for future use by contacting us at support@intellxxdna.com.
Can I disguise how my identity appears in the system?
Yes. Your clinician can choose to use a name or identifier that is not your full or real name when registering you in our system. You must request your clinician or clinician’s office to do this at the time that they first request IntellxxDNA services for you.
If you have already been entered into our system under your real name, you may request that we use an alias going forward. However, a name change request may not delete the name originally used for your account in historical, archived or other data storage resources.
