IntellxxDNA™, LLC (“IntellxxDNA”, “we” or “us”) provides personalized genomics analysis services to health care professionals (clinicians) in order to help clinicians support and personalize clinical recommendations for their patients. This Privacy Policy describes how IntellxxDNA collects, uses, process, discloses, stores, and protects the Personal Information we receive and collect about you when you visit our website at https://intellxxdna.com/ (the “Website”).
The purpose of this Website is to provide information to clinicians and individuals who are interested in our services. No genetic information is collected, stored, or accessible through this Website. This Privacy Policy does not govern our privacy practices relating to the handling of patient personal health information, including genetic data; genetic data is accessible only through our Patient Portal, which is at a separate website address.
You create an account on the Patient Portal account only if and when you order a saliva test kit. The saliva kit may be ordered only from clinicians. Your clinician will share your test results with you through the Patient Portal.

“Personal Information” is information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular individual.
- Why We Collect and Use Your Personal Information / Purposes
We collect Personal Information from you so that you can access and use our Website and so that we can communicate with you to respond to your requests (i) for information through the forms you fill out or (ii) to create a Clinician Account (if you are a licensed health care professional).
You do not have to register to visit or use our Website. However, we do collect some Personal Information when you visit our Website. Please see “Information We Collect Automatically When You Use the Website,” below, for detailed information regarding the Personal Information we collect when you visit and use our Website, and the choices you have with regards to that collection. We may monitor use of the Website in order to detect, investigate or prevent any actual or potential violation of our terms of service. We may also generate aggregated, non-identifiable data and use such information to monitor the performance, use and stability of the Website and to improve the Website.
- Information You Provide Directly to Us
- Connect to a Clinician: If you ask to be referred to a clinician, we will collect your name, email address, phone number, country, postal code as well as your interest in our services for the purpose of informing you about clinicians that are located near you.
- Contact Us: If you are a clinician and ask through the Website for a Clinician Liaison to contact you, we will collect your name, email address, phone number, practice name and license type for the purpose of responding to your request. Calls may be recorded to ensure quality of service to you and for training purposes. We may retain this information to assist you in the future and to improve our customer service and service offerings.
- Create a Clinician Account: If you are a clinician and ask to create an account, we will collect and use your full name, job title, practice name, practice type, practice URL (on an optional basis), address, city, province/state, country, postal code, email address, phone number, license number, issuing state/province, national provider identification number (including issue and expiry dates), license, referrer details (if applicable) and any other information you choose to provide us when requesting an account. We collect this information for the purposes of creating and administering your account and facilitating your access to and use of our services.
- Information We Collect Automatically When You Use the Website
- Information We Need to Display Webpages to You. Our servers automatically record your visits to the Website. Like many other website servers, ours compiles a log that records the date, time, browser type (i.e., Firefox), device type (i.e., iPhone), operating system (i.e. Android), and originating IP (Internet protocol) address of each page you visit. We need this information to show you the webpages you want to see on our Website.

- Clinicians: If you ask to be connected with a clinician who has partnered with us, we will share your [full name, email address, phone number, country, postal code and interest] with a clinician located near you who can help you access our services.
- Service Providers:Your Personal Information will be transferred (or otherwise made available) to certain third parties that provide services on our behalf. We use service providers to provide services such as providing IT support, hosting the Website, conducting analytics, and providing advertising and marketing services. Our service providers are only provided with the information they need to perform their designated functions and are not authorized to use or disclose Personal Information for their own marketing or other purposes. We and our service providers may access, store and otherwise process Personal Information of the United States.
